Our Prosperiti Care Support Program is here to support you. We offer community-based services to people who have a disability and their families to support them to participate in meaningful and preferred ways with their communities.

Here at Prosperiti Care Support we are all about making your NDIS Plan work for you. We want to understand what you want your NDIS to look like. What’s important to you? What could we do together?

In the Perth metro area, Peel, South West and Great Southern regions, we have local supervisors who already know the area you live in and can help you translate your NDIS plan to build the individual support you decide on.

We’ll support you through the process of understanding your plan and choosing which supports will best help you reach your goals. Most importantly we’ll lean from you about what’s important to you and what you need so that we can provide you with the best possible service.
